OSF HealthCare is an integrated health system consisting of 11 acute care facilities with a total of 1,540 licensed acute care beds and two colleges of nursing. OSF HealthCare had a system-wide strategic initiative to provide palliative care, including advanced care planning (ACP). Their goals were to identify the population, provide outreach to them for ACP, and compare costs of care and alignment of care with expressed desires. OSF HealthCare used Informatica PowerCenter to integrate data from claims, registry and electronic medical records (EMR) systems to provide a 360 degree picture of high risk patients. Cross-referencing this data enabled OSF HealthCare to target outreach to patients and families to initiate ACP discussions. OSF HealthCare achieved their goal to connect with 1,000 patients in three quarters. They attribute success to every day reporting on progress which provided actionable data. Medical and clinical administrators championed the results because when patient and family-centered goals of care are aligned with actual care delivered, OSF HealthCare can improve quality, patient satisfaction and reduce cost.
